The same problem here with my Bluetooth mouse. My adapter information is:
Bus 003 Device 004: ID 0a5c:201e Broadcom Corp. IBM Integrated Bluetooth IV

A weird behavior observed: when the mouse can not reconnect, toggle the
"Visibility setting" in "Bluetooth Preferences" dialog from "Hidden" to
"Always visible" will help the bluetooth mouse get re-discovered.
