There are not that many you know.  And prices are so aggressive today
that it is very hard for small/medium structures to challenge the big
actors of the market.  Especially on their former local market.

Moreover, for personal use, what you mostly want is a free (as beer)
computer-to-computer phone call ... This is a direct connection between
both computers so the provider is not really meaningful.

My biggest problem with SIP is that is far too complex for
non-computer-freak users.  It is not a firewall and NAT-friendly
technology and you have such equipment and configuration
everywhere!
