v86d clash

Hi Ben,

I've rejected your v86d upload in favour of a sync from Debian, figuring
that we should start out from a common base. However, there were parts
of your upload that aren't in Debian, most notably the initramfs hook,
although uvesafb.h may need to be added as well. Could you look at the
package that will be in universe shortly and merge your work into that?
