Ubuntu Accessibility Start Guide update

I have updated the Start Guide at
https://help.ubuntu.com/community/Accessibility/doc/StartGuide .
Hopefully, this is now a useful document to refer users to. Since the
latest release that allows an installation using the screen-reader is
Ubuntu 10.04, that is what I will be referring to in most of the
documentation. Users can always install Ubuntu 10.04, then upgrade to
10.10. 

Please let me know of any errors in the guide, so that I can get them
corrected as soon as possible. Or, please feel free to update the page
so that we can keep it usable for users that need that information.
