I must admit that this is above my level of knowledge, but I will try to
look for some clues ...
Are you trying to create 14.04.5 with the trusty kernel?
mini-14.04.1.iso has the trusty kernel 3.13.0-92, which is written in
the beginning of your dump:

mini-14.04.5.iso has the xenial kernel 4.4.0-31.
Maybe something is incompatible between 14.04.1 and 14.04.5?
On the other hand, I could install both Lubuntu point versions from the
two mini.iso files.
If you do the work from a host system running Xenial, maybe it would
work better. See also the information about the LTS enablement stacks of
the point releases,

that might give some clue about what may go wrong.
