CM108 USB Audio device stopped working

http://imagebin.org/318368 is a screenshot of pulseaudio volume
control showing that Amaok is using the CM108 controller.
http://imagebin.org/318369 also shows the output to the CM108
controller. I've given the highest priority to the CM108
controller(http://imagebin.org/318371) and these settings did work
until recently. Also, when I play a song in Amarok, the light on the
USB device blinks continuously so I assume the data does reach the
device.
