Now the question is, what is your assumption?

1) Update-manager was faulty when the link /dev/dvd -> /dev/hda 
became /dev/dvd1 ->/dev/scd0.

2) Some people have used the unsupported upgrade path with another package 
manager and that didn't take care of the problem.

I think both are possible. When Hardy was released, a neighbour upgraded 
several machines always using update-manager. IIRC he had the problem of 
the broken link /dev/dvd1 -> /dev/scd0 on those machines. And people 
using the unsupported path sure didn't know about the problem.
