Kaffeine, Xine and kscd fixed in Hardy by /dev/scd0

Rod, It's not really a fix because it wouldn't fix the problem for those
with multiple optical drives.  The solution involving the udev rules is
more elegant, and probably involves fewer steps, i.e. simpler, and yet
more effective.
