How do I get Kmail to show on the Kmenu/Internet dropdown again?

so you know it is on your system and does activate.

Those smaller icons are in the quicklauncher panel IIUC.

You cannot have larger icons on the quicklauncher AFAIU, not 
a mix of large and small anyway.

The easiest way I have found to add just a normal size icon 
to a kpanel is to create the icon on the desktop first. 
Right click on your desktop, create new link to 
application, choose the application tab, fill in 
description with your description. fill in the command line 
with kmail or /usr/bin/kmail, normally just kmail would 
work for my system. Select the General tab and choose your 
icon.

when that is done just drag and drop it onto the kpanel. 
Make sure it *is* a kpanel you are dropping it onto and the 
panel is not locked. If it is locked right click on a blank 
bit of the panel and unlock it first.

I just did all that and it works on my system.
