My usual ugly hack way to kill something like this is to

sudo chmod -x /etc/init.d/foo

If it's not executable, it stops running.  That's not the Correct 
Debian/Ubuntu Way(tm) but it works.  Sometimes you have to just hit something 
with a big hammer to make it shut up.
