Well that sucks :-(

So:

Your output from "iwconfig wlan0" is identical to mine

"dmesg | grep ndis" is slightly different:
- you're not using the stock ndiswrapper from gutsy (which is 1.45 - or at
least, that's what it reports, while apt says it's 1.43).  That's probably
not a problem, but I know 1.45 works.
- you have exactly the same Windows driver ("bcmwl5 (Broadcom,10/12/2006,
4.100.15.5) ")

The only difference from "dmesg | grep wlan0" is that your ndiswrapper
reports "vendor: 'NDIS Network Adapter'", while mine reports the vendor as
null.

And finally lspci shows the same (except that the USB port is different).

What's in your /etc/network/interfaces?
