upgrade and dist-upgrade only look at already installed packages.  It
will not install so-called "missing" packages.  It doesn't have a
master list of what you originally installed.  All the upgrade and
dist-upgrade targets do is go through the list of packages you have
installed that have upgrades available, work out the dependencies of
those upgrades, and then upgrade/install those packages.

The difference between upgrade and dist-upgrade is that one tries
harder to not uninstall existing packages, and to work out the
dependencies better.
