Fixed some libqapt issues

Hey all, I found and squashed the issues involving source entry duplication
in libqapt. There were two problems. One was that you could add the same
source file to the list twice. The other is that when reloading the file,
it wasn't actually clearing the entries in memory before reloading the file.

I have added automated tests to cover more of these cases, and I have
committed these to the master.
