I am running Lubuntu 16.04.3, 
These are the steps which fixed the icons on dolphin for me:

1) sudo apt-get install oxygen5-icon-theme libqt5libqgtk2
2) Menu -> Preferences -> Customize Look and Feel -> Icon Theme -> Click on Oxygen -> Apply
