Confirmed on a fully updated Kubuntu Hardy (recently upgraded from
Gutsy) with Firefox 3 running the Plastik KDE theme.

Installing the gtk2-engines package provided a number of Gtk+ themes.
Then, going into "System Settings", under "Look & Feel" choosing
"Appearance", then entering the "GTK Styles and Fonts" panel and
choosing Clearlooks in the "GTK Styles" section did provide properly
functioning form controls once I restarted Firefox.

I decided to install gtk2-engines-qtcurve and choose QtCurve both as my
KDE style and as the GTK+ style described above. However, the GTK+ style
must be chosen explicitly: the "Use my KDE style in GTK applications"
does not manage to properly enable the QtCurve styling for Firefox. This
is itself probably some kind of bug or omission.

These kinds of bugs are awkward because although one shouldn't override
the user's theme choices when upgrading their system, one should avoid
leaving them with inadequate themes or configurations.
