Using xev I could understand keys were mapped wrongly for evdev or default xkb keycode files.
I have fixed this for me by editing /etc/X11/xkb/keycodes/evdev, /etc/X11/xkb/keycodes/xfree86, /etc/X11/xkb/keymap/xfree86, /etc/X11/xkb/symbols/br, /etc/X11/xkb/symbols/inet, /etc/X11/xkb/symbols/jp (conflict) and finally /etc/X11/xorg.conf and gconf-editor (desktop, gnome, peripherals, keyboard, kbd).
