Just to add some fresh info on the subject:
In current Gutsy, the problem is still present. In addition, all PDF related applications seems to have the same problem. I tried kpdf as well as Acrobat reader and even printing a pdf from the command line using lpr. All have the same behaviour of printing in portrait mode and with the truncated output. lpr prints in landscape when passing the "-o landscape" command line option, but the printing is useless anyway because the margins are way off.

I can confirm that the pdf attached by Luke (booking_form_2006.pdf)
still works flawlessly.
