SRU Justification

[Impact]

The PCI and queue number info is missing in MANA IRQ names. request_irq() will
fail if the name is incorrect.

[Test Case]

Load the MANA module and ascertain if it detected a device.

[Where things could go wrong]

Existing instances could fail to initialize.
