[c, b, x][PATCH 0/3] LP: #1834499 - Block Layer Commits Requested in Azure Kernels (for the *generic* kernels)

Those fixes were requested for the azure kernels but theys are
relevant for all kernels, so apply them to the master kernels.

Those are simple patches and clean cherry picks from upstream, the
first patch (98a09d6106660 - "bio_copy_from_iter(): get rid of copying
iov_iter") is only need for 4.4.

Also, since they were introduced upstream in 4.20, they are not needed
for Disco.
