[PATCH 0/3][SRU Cosmic][SRU Bionic] iommu/arm-smmu-v3: Don't disable SMMU in kdump kernel

We fixed this issue for disco in the last SRU cycle, but since discovered
it also impacts Cosmic & Bionic. Just one patch (1/3) is needed for Cosmic -
Bionic requires an additional backport (2/3 & 3/3). (2/3) required minor
backporting, described in the commit footer.
