[CVE-2017-7346][T/X/Y/Z/A SRU] drm/vmwgfx: limit the number of mip levels in vmw_gb_surface_define_ioctl()

This upstream patch can be cherry-picked for X/Y/Z/A.

For Trusty, there are several lines have been changed in other commits (294adf7, 233826a7 and 53c1e53), so I ended up just backporting the change in this upstream patch.

Both Precise-ESM and Vivid are skipped as this is a medium CVE.
