[PATCH 0/2][CVE-2016-3961] hugetlbfs use may crash PV Linux guests

Following this email I'm sending the fix for Xen Security Advisory
CVE-2016-3961 / XSA-174.  It's a clean cherry-pick for all kernels, except
for Precise, that required a minor context adjustment.
