[PATCH 0/2][CVE-2016-2187] Kernel panic on invalid USB device descriptor (gtco driver)

Following this email I am sending the fix for CVE-2016-2187.  It's a clean
cherry-pick for all kernels, except for Precise that required a small
context adjustment.
