[Lucid][CVE-2012-2121 0/1] KVM: unmap pages from the iommu when slots are removed

Following this email, I'm sending CVE-2012-2121 fix backport.  This
fix has been picked from the Debian 2.6.32-48squeeze4 kernel, which
has been backported by Ben Hutchings.

Note that this patch didn't contain Ben's SOB.  Obviously, I didn't
added it, I just added my own SOB.
