[Lucid][CVE-2013-4299 0/1] dm snapshot: fix data corruption

Following this email there's a patch that contains a backport of the fix
for CVE-2013-4299.  It's a simple backport that, appart from context
adjustment, replaces the constant 'NUM_SNAPSHOT_HDR_CHUNKS' by '1'.
This constant was introduced by commit

 4454a621 dm exception store: add merge specific methods

which is too intrusive to backport to Lucid.
