[Quantal SRU] USB bug fixes

These patches are suggested by Sarah Sharp to be backported to older kernels like 3.5. So far, upstream stable kernel 3.5.y doesn't include these patches, please consider to include them to Quantal.

1.      OSVs that backported the command cancellation patches (necessary to avoid a kernel crash on repeated plug/unplug of a VIA hub) should also backport this command cancellation fix:
43a09f7 xhci: Fix potential NULL ptr deref in command cancellation.

2.      OSVs that backported the quirk to fix the "dead ports" issue on HP IVB/PPT systems (commit 71c731a296f1b08a3724bd1b514b64f1bda87a23 "usb: host: xhci: Fix Compliance Mode on SN65LVPE502CP Hardware") should also pick up this patch to extend the quirk to new HP system models:

3.      USB3 Link Power Management (LPM) was included in Quantal, and these patches are the bug fixes for LPM:
   d01f87c Enable LPM after a failed probe
   ae8963a usb: Don't enable LPM if the exit latency is zero.
   65a95b7 usb: Send Set SEL before enabling parent U1/U2 timeout.
   966e7a8 xhci: endianness xhci_calculate_intel_u2_timeout
   16b45fd xhci: fix integer overflow
