[PATCH 0/6] kms blacklist updates

We had a number of very late emerging requests to blacklist KMS on Intel
i8x0 and on ATI ES1000 cards.  Those were tested both on affected and
non-affected hardware.  These patches were applied and uploaded in
2.6.32-21.32.  Posted here for completeness.
