UBUNTU: [SAUCE] Update Poulsbo Video driver to build 5.0.0.0040, Take 2

This patch is required for LPIA image build.  Steve Magoun has tested
the packages built from this patch for testing.

After reviewing the patch itself per comments, it is not clear that
splitting the patch would benefit.  There was confusion over
the defines DVD_FIX and PSB_DETEAR.  These are one and the same,
just re-factored in the process from development to production.  The
individual file changes all relate to the same detear functionality fix.
The code changes themselves complement the changes in the
video driver for X.

The formatted patch with an expanded commit log message is attached.

Note that this patch is paired with patches applied to the X server.
The patch was supplied/referenced in the bug report.
