Wiki page on bug fixing

I've created a wiki page to document the bug fixing process as I understand it 
from our meeting in Portland last week.  This has been both an exercise in
learning by documenting and an attempt to get this part of our process

All errors are the fault of my semi-non-volatile memory and muddled thinking.
