Problem also occurs if USB is plugged in to Dell 2405FPW monitor with built-in
USB hub / card reader.
identifies as:
 ID 0424:223a Standard Microsystems Corp. 8-in-1 Card Reader

This one is more annoying to unplug at boot since its the hub for devices as well.
