ipw2200 has been working fine for me with all recent kernels.

The problem described in comment #5 is a different issue; I've seen that as well
(usually after suspend).

The firmware is not suppossed to be in /usr/lib/hotplug/firmware, but
/lib/hotplug/firmware (where it is).  One guess would be that you installed a
non-Ubuntu hotplug package which is not searching in the same locations.

dpkg -l hotplug linux-image-`uname -r`
