Public bug reported:

My nightly backup script creates snapshots of volumes. The snapshots
look good and are accesible, but error message is displayed when
lvcreate lvremote is invoked:

  /dev/vg00/dynamic.nethome.lv-disk-snap: read failed after 0 of 4096 at 4294901760: Input/output error
  /dev/vg00/dynamic.nethome.lv-disk-snap: read failed after 0 of 4096 at 0: Input/output error
  /dev/vg00/exp-disk-snap: read failed after 0 of 4096 at 4294901760: Input/output error
  /dev/vg00/exp-disk-snap: read failed after 0 of 4096 at 0: Input/output error
