This could possibly be a manifestation of this upstream bug:

I'm not going to link to the upstream bug until we're sure. In the
meantime, I'll try to get a test kernel built with the two patches known
to fix the problem, and see if they solve it for you.

Your test case is valuable, as the upstream bug is random and not easily
reproduceable.
