[Bug 626261] [NEW] Video but no sound in default LTS

Public bug reported:

Both Konqueror nor Firefox are able to play video, but neither produce
the accompanying audio.  I would expect that the sound for at least
system notifications work out of the box.  The sound tests in
Settings->System settings->Multimedia show Nvidia HDA ALC889A  but
neither the digital nor audio icons produce sound even with the test.
Jack Audio Connection kit produces an error when it is tested.

For example this presentation shows video in both, but no sound:

I've tried various Volume levels in the system and applications.
I've tried both internal speakers and the audio out jack.  

This is in a fresh installation of LTS 10.4.1 Kubuntu from CD updated
via APT and the online repositories.

"ubuntu-bug audio" only works with Pulse Audio, which is not part of the
default LTS installation

The following from the ubuntu wiki page "Debugging Sound Problems" produces no output:
