Case 1 is now (possibly) invalid. I had the following line in my fstab:

none /proc/bus/usb usbfs devgid=126,devmode=664 0 0

I don't actually have a usbfs (AFAIK), so I commented this line out (due
to the behaviour noted in 553290).
