I'm seeing a symptom of the same problem.
I created a 9.04 install CD using preseeding to make a root LVM partition. It worked fine until I added a step to update the kernel. The resultant initrd cannot boot from the LVM root partition.
If I use non-LVM partitions for the root it's OK, but with an LVM root
it drops to the (initramfs) prompt and there under /dev/mapper there is
only the control file, none of the LVM partitions are visible.
