I had the same problem after upgrading to Karmic from Jaunty. Always end up with BusyBox shell. 
 /proc/modules contains the following infomation:

thermal
processor 
fan
fuse
dm_mirror 27008 0 - live ..
dm_log 17924 1 dm_mirror, Live 0x...
dm_mod 63432 2 dm_mirror, dm_log, Live 0xxxx

I had a IBM Xseriers 235, two SCSI disks with no RAID setting.
