[Bug 118353] Grub installation on LVM (new installation, kernel update) fails after lvrename

Public bug reported:

Binary package hint: linux-image-2.6-686

* When installing a new kernel image (dist upgrade, kernel update) ...

* on a LVM system where the root logical volume had been renamed via
lvrename,  ...

* the kernel entry in grub's menu.lst is written with "root=OLD_LV_NAME"
instead of the new LV name.

* The system does not boot because the kernel does not find its root
device.

This happened several times when updating the kernel in 6.06 and 7.04 as
well as during the upgrade from 6.06 to7.04 on at least two different
systems.
