I have exactly the same laptop and the same symptoms: HP Pavilion zv5405us and
none of the usb ports are working (2 x 1.1, 1 x 2.0). The USB ports are working
on winxp home and on debian pure64. The mouse I used to test the USB ports is
working on another laptop (Dell latitude d505) with hoary and breezy.

lspci says:
