don't forget the help

There's been at least a couple features which should have modified the juju
command line help docs and did not ..... please remember to update the help
docs, or no one will know the feature exists.

Cases in point:

juju help constraints was not updated when we added the instance-type
constraint.
(I just fixed this one)

juju help upgrade-juju still says that odd minor versions means development
releases.

This is pretty important stuff, especially if the help is just plain wrong
(as above).
