juju, maas, ceph-mon, cloud-init: /etc/hosts overrides DNS, ceph doesn't listen on 127.0.1.1

commands like sudo are also upset by the missing host name in
/etc/hosts. We have updated our own charms and provisioning scripts to
add this as a work around
