  Upgraded 12.10 to 13.04 and boot will not prompt for disk encryption

Bug description:
  I have a Thinkpad W520 laptop that was running 12.10 and full disk
  encryption and software updater offered upgrade to 13.04. After about
  an hour, the upgrade completed and system rebooted only to not ask for
  passphrase and system drops to busybox shell since the / LVM volume
  wasn't found. Fortunately, I still had the older 3.5.0-27 to choose
  from grub menu and it prompts for passphrase and I can boot the system
  this way but attempting to boot the 3.8.0-19 kernel never prompts for
  passphrase.
