It's possible you had a bad write to your installation media or for some
other reason its contents could not be read correctly at this time. If
you're still having problems you should try re-writing the image to your
media (and may be re-download the image as well).

Disk error during live install
Status in ubiquity package in Ubuntu:

Bug description:
After following steps to install from the 'Install (Live session) -
(Legacy)...' Testcase on iso.qa.ubuntu.com I ran into an error with
the disk after the password is set.
Files were being copied for install and the <Welcome to Ubuntu> slide
was up. I haven't had issues with installing from this media before.
There was a GTK error early on but everything worked properly after
that.
