It's possible you had a bad write to your installation media or for some
other reason its contents could not be read correctly.

I'm closing this report as this wasn't a bug with the Ubuntu installer.

  I had attempted to install to an encrypted LVM, which failed, then
  tried to install with the default partition scheme, at which point the
  whole installer crashed.
