It's possible you had a bad write to your installation media or for some
other reason its contents could not be read correctly.

Ubuntu 16.04 (xenial) reached end-of-standard-support on April 29, 2021.

Please only install a supported release of Ubuntu. See this document for
currently supported Ubuntu releases: https://wiki.ubuntu.com/Releases
