I think it is unlikely that the hard drive issues you are experiencing
are related to ubuntu-release-upgrader.

I suspect a Support site would be more appropriate, eg.

Bug description:
  No live CD could read the hard disk, but the other drive is still
  working.

  The up grader said during upgrade that a partial upgrade took place
  continue or not. later a release upgrade could be done.
