ie. problems wth device sda were encountered. I'd suggest checking the
health of your drive (using it's SMART,
https://help.ubuntu.com/community/Smartmontools), maybe running
`badblocks` etc.

Maybe seeking support may help, as it appears to be hardware issue to me
