Ok after re-writing the installation media I ran the integrity check and
it returned no errors and the integrity check passed.
Not sure if it is related but the installation crashed again with OOPS-
a192d32a11c847d4b98b1c194610dba5
It was further into the install when this error occurred so I'm not sure
this is relevant to this issue. At this point I am considering that the
usb I have is in some way damaged. I am not sure where to find this bug
report? (possible lliurex-ubiquity-casper/+bug/998492)

Status in ubiquity package in Ubuntu:
Incomplete
Bug description:
While installing a few seconds after the installation to disk starts,
the process fails with "ubi-usersetup crashed".
All was working until the process began writing to the disk and then
the failure occurred.
Options selected during install are as follows:
- lvm
- luks
(possible errors from faulty hardware or usbs)
