Further to comment #2 did you resolve this issue by re-downloading the
image, re-writing the image and checking for disk defects?

  Crash in install

Status in ubiquity package in Ubuntu:
  Incomplete

Bug description:
  On the screen where partitions are entered, I forgot to select
  "format" the partition that already had Ubuntu 18.04 LTS. The install
  alerted and I returned to select. After that, an error message popped
  up saying I had a problem with the HD / CD.
