Thank you for your bug report, but if as you report the disk is having
issues then it's not a software bug.
The report also include the information
CasperMD5CheckResult: fail
Which indicates the image downloaded or the USB Key are corrupted, you
should try to download and write it again

very frequent errors appears when i try to boot :fsck forced on /dev/sda5 and then exited with status 1 but no way to boot."
trying to repair with USB key "LiveCD"
but i got message that explain impossible to keep actual system partition.
I have to reinstall Ubuntu after reformat entire hard disk drive.
All data will be lost.
